Can You Still Use Chinese Medical Insurance After Becoming a Foreign Citizen? Three Situations Explained
Naturalised after retiring: medical insurance continues as normal
If you acquired foreign nationality only after lawfully retiring in China — that is, you were already retired when you became a foreign national — your Chinese medical insurance and social security card can still be used. You can come back to China for treatment, settle bills with your social security card and enjoy the same medical insurance benefits as before.
Naturalised before retiring: participation ends automatically
If you acquired foreign nationality before retiring, your participation in resident or employee medical insurance ends at the moment you acquire foreign nationality, and medical costs incurred in China afterwards can no longer be reimbursed in real time. However, the balance previously accumulated in your personal medical insurance account is personal property and can still be used.
The grey area: hukou not cancelled, foreign nationality unknown to the system
If after naturalising you did not cancel your Chinese hukou and continue to pay social insurance, for example as a flexibly employed person, then while the medical insurance agency is unaware of your change of nationality, your card may formally still work. But whether this is lawful is disputed, and it should not be treated as a safe route.
The legal risk of medical insurance fraud
Under current rules, obtaining payments from medical security funds by deception, concealment or similar means can lead not only to recovery of the money but also to administrative penalties and even criminal liability. The crackdown on medical insurance fraud keeps intensifying, and convictions for such conduct are not rare.
Our advice
After acquiring foreign nationality, find out about and change your insurance status as required, to avoid unnecessary legal risk caused by an information gap.
